Job Summary:

Norfolk County Council is seeking a skilled and dedicated Integrated Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare services.

Key Responsibilities:
