About the Role

We are seeking a skilled and compassionate Community Rehab Assistant to join our team. As a key member of our Intermediate Care Team, you will work closely with senior team members to provide high-quality care and rehabilitation to individuals in their own homes.

Our goal is to prevent unnecessary hospital admissions and provide a seamless transition from hospital to home. You will be responsible for delivering care and rehabilitation services that meet the unique needs of each patient, working collaboratively with our therapy team to develop and implement personalized care plans.

Main Responsibilities:

  • Provide rehabilitation and care services to patients in their own homes, working closely with the therapy team to develop and implement care plans.
  • Assist with home exercise programs as prescribed by the therapy team, promoting patient independence and mobility.
  • Maintain knowledge and skills necessary to deliver high-quality care, staying up-to-date with best practices and guidelines.
  • Provide clerical and administrative support to the team, ensuring accurate and timely documentation.
  • Deliver care in accordance with organizational policies, procedures, and guidelines, adhering to competencies and delegations from the OPS Lead/Therapist.
  • Report any changes in patient condition to the Operational Lead, ensuring timely interventions and adjustments to care plans.

Requirements:

  • NVQ level 3 or equivalent qualification/experience, with a strong foundation in care and rehabilitation principles.
  • Excellent communication and organizational skills, with the ability to work autonomously and as part of a team.
  • Flexibility and adaptability, with a willingness to work in a fast-paced environment and respond to changing patient needs.
  • Confidence in using IT systems, including Microsoft Office packages and EMIS.
  • Valid driver's license and access to a reliable vehicle, with the ability to travel across our services in a timely manner.

What We Offer:

  • A competitive salary of £22,308 (pro-rated to hours worked), with the opportunity to earn a group pension.
  • Plentiful on-site parking and free tea and coffee at your base location.
  • Membership of My Reward Hub, offering discounts on everyday purchases, cashback, and voucher offers.
  • Access to your wages as you earn them, helping you cover life's emergencies and avoid overdraft fees.
  • Online and face-to-face support for mental and physical wellbeing, including healthy recipes, activity challenges, and post-trauma support.
  • Access to eLearning, bespoke career pathways, and opportunities for continuing professional development through our 'Outstanding' learning and development team, The Learning Enterprise.
  • An open, just culture where you're encouraged to share ideas and contribute to our purpose: changing lives through transforming health and care.
