Our goal is to prevent unnecessary hospital admissions and provide a seamless transition from hospital to home. You will be responsible for delivering care and rehabilitation services that meet the unique needs of each patient, working collaboratively with our therapy team to develop and implement personalized care plans.
Main Responsibilities:
- Provide rehabilitation and care services to patients in their own homes, working closely with the therapy team to develop and implement care plans.
- Assist with home exercise programs as prescribed by the therapy team, promoting patient independence and mobility.
- Maintain knowledge and skills necessary to deliver high-quality care, staying up-to-date with best practices and guidelines.
- Provide clerical and administrative support to the team, ensuring accurate and timely documentation.
- Deliver care in accordance with organizational policies, procedures, and guidelines, adhering to competencies and delegations from the OPS Lead/Therapist.
- Report any changes in patient condition to the Operational Lead, ensuring timely interventions and adjustments to care plans.
Requirements:
- NVQ level 3 or equivalent qualification/experience, with a strong foundation in care and rehabilitation principles.
- Excellent communication and organizational skills, with the ability to work autonomously and as part of a team.
- Flexibility and adaptability, with a willingness to work in a dynamic and fast-paced environment.
- Confidence in using IT systems, including Microsoft Office packages and EMIS.
- Valid driving license and access to a reliable vehicle, with the ability to travel across our services in a timely manner.
