Responsibilities of the Role

The Community Health Support Worker will be tasked with conducting health assessments for specific groups within the community and directing them to various support and educational resources related to healthy living.

Established protocols for escalation will be adhered to, with remote assistance available from the management team and a Supervisory GP.

Health assessments will encompass measurements such as height, weight, and BMI, alongside cholesterol testing and guidance on healthy lifestyle choices.

Key Responsibilities

Purpose of the Role

The Community Health Support Worker will undertake health assessments for targeted individuals in the community and guide them to appropriate support and educational resources regarding healthy living. Specific pathways for escalation will be followed, with remote support from the management team and a Supervisory GP.

Main Duties

  • Execute and document health assessment tasks according to established guidelines, including:
  • Blood pressure measurements
  • Heart rate assessments
  • Height and weight evaluations
  • Cholesterol testing
  • Providing education on healthy living
  • Utilizing EMIS web for accurate consultation notes
  • Daily logging and quality control of testing equipment
  • Ensuring compliance with sharps and clinical waste management guidelines
  • Recognizing the need for urgent care and taking appropriate action
  • Supporting individuals by providing information on relevant health services

Qualifications and Experience

Desirable Qualifications

  • NVQ Level 1 & 2 in Health & Social Care
  • Care Certificate (new to Practice from April 2015)
  • Training in wound care and phlebotomy
  • Current CPR certification (within 18 months)

  • Experience in patient consultations
  • Understanding and adherence to confidentiality protocols
