Position Overview

Are you dedicated to the field of rehabilitation and committed to assisting individuals in achieving their independence and functionality within their own living environments?

We are seeking a Rehabilitation Support Practitioner to join our collaborative Occupational Therapy team, working alongside licensed therapy professionals to provide exceptional rehabilitation services to clients in their residences.

The therapy team is composed of Occupational Therapists, Physiotherapists, Rehabilitation Support Practitioners, and Therapy Assistants.

In this role, you will manage a caseload assigned by the designated clinician, focusing on delivering interventions to patients. You will evaluate each individual's functional capabilities and assess their daily living activities.

Your responsibilities will include designing and adjusting treatment plans while collaborating closely with team members to ensure optimal therapeutic outcomes for your clients.

We are looking for candidates who possess a genuine interest in supporting individuals and demonstrate enthusiasm and motivation in helping them regain and maintain their independence. The Community Therapy team serves a broad area, and this position will primarily focus on delivering Occupational Therapy services.

Key Responsibilities

The successful candidate will be an integral part of the Community Therapy Team, providing assessments and rehabilitation as part of a comprehensive multi-disciplinary service.

Your duties will include:

  • Conducting assessments and treatments for patients with varied health conditions, primarily in their homes, with the flexibility to work in other settings as necessary.
  • Providing feedback to the qualified Occupational Therapist and recognizing your own professional limitations.
  • Engaging in regular informal and formal supervision sessions.
  • Assessing, providing, and verifying the delivery of aids, equipment, and exercises for your assigned caseload.
  • Managing a delegated caseload daily, including prioritizing tasks effectively.
  • Working independently in the community in accordance with established policies regarding lone working.
  • Adhering to the ethical standards and professional conduct guidelines relevant to Occupational Therapy and Allied Health Professionals.

The Community Therapy team operates across a wide region, and the successful candidate will be based in the community, providing rehabilitation services to individuals in their local area.

It is essential for the candidate to have access to transportation to facilitate home visits.

Qualifications & Experience

Essential:

  • Foundation Degree in Health and Social Care or an equivalent qualification, or a commitment to pursue this qualification within a specified timeframe.
  • Full UK driving license and the ability to travel independently.
  • Proficient computer skills relevant to the role.
  • Literacy skills at level 2 or above.

Desirable:

  • Training related to Occupational Therapy or Physiotherapy core competencies.

Experience:

Essential:

  • Experience in an Occupational Therapy, Physiotherapy, or related health service.
  • Ability to make independent clinical decisions at the appropriate level.
  • Experience managing a personal caseload and seeking support when necessary.
  • Knowledge of manual handling practices and relevant legislation.
  • Proven communication skills.

Desirable:

  • Experience in a community health environment.
  • Commitment to ongoing professional development.
