**Job Overview**

We are seeking an experienced and motivated Occupational Therapist to join our team at NELFT NHS Foundation Trust. As a key member of our Integrated Community Team, you will play a vital role in providing high-quality patient-centered care to individuals with physical conditions, including neurological, orthopaedic, falls, long-term conditions, and musculoskeletal issues.

**Key Responsibilities**

  • Work closely with community nurses, community matrons/care coordinators, physiotherapists, occupational therapists, and rehabilitation assistants to maintain independence, promote self-care, and assist patients in achieving their goals.
  • Conduct thorough assessments and develop personalized rehabilitation plans to address patients' physical and functional needs.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Provide education and support to patients, families, and caregivers to promote healthy behaviors and self-management skills.
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practice guidelines.

**Requirements**

  • Experience working at B5/B6 level in a community rehabilitation setting.
  • Previous NHS experience, preferably in occupational therapy.
  • Degree in Occupational Therapy or equivalent qualification.
  • HCPC registration.
