Job Title: Physical Health Nurse Specialist

Salary: £36,000 - £38,000

Shift: Day Shifts (Monday to Friday, 9:00-5:00)

Hours per week: 40

Key Responsibilities:

  • Triage and Coordinate Care: Triage GP referrals and coordinate weekly clinics to ensure seamless patient care.
  • Collaborate with Healthcare Professionals: Liaise with healthcare professionals to ensure effective patient care and treatment.
  • Monitor Patient Health: Monitor patient health issues, including obesity, diabetes, epilepsy, and asthma, and develop strategies for prevention and management.
  • Conduct Health Reviews and ECGs: Conduct regular health reviews and ECGs to assess patient health and identify potential issues.
  • Run Vaccination Clinics: Run vaccination clinics for patients and staff to promote health and prevent illness.
  • Maintain Medical Records: Maintain accurate and detailed medical records to ensure continuity of care.
  • Develop Audits and Action Plans: Develop audits and implement action plans to improve patient care and outcomes.
  • Contribute to Team Development: Contribute to the induction of new team members and provide guidance and support to ensure a smooth transition.

  • Registered Nurse Qualification: Registered Nurse Qualification (RGN)
  • Valid NMC Pin: Valid NMC Pin
  • Clear DBS: Clear DBS
  • Proven Track Record: Proven track record of delivering person-centered care, including care planning
