Job Title: Physical Health Nurse

Job Summary:

We are seeking a highly skilled and dedicated Physical Health Nurse to join our team at Find Medic. As a Physical Health Nurse, you will play a vital role in providing exceptional care to women with complex mental health conditions.

Key Responsibilities:

  • Triage and Coordination: Triage GP referrals and coordinate weekly clinics to ensure seamless patient care.
  • Health Monitoring: Monitor patient health issues, including obesity, diabetes, epilepsy, and asthma, and conduct regular health reviews and ECGs.
  • Health Promotion: Run vaccination clinics for patients and staff, and maintain accurate and detailed medical records.
  • Quality Improvement: Develop audits and implement action plans to improve patient care and outcomes.
  • Team Collaboration: Liaise with healthcare professionals to ensure effective patient care and contribute to the induction of new team members.

Requirements:

  • Qualifications: Registered Nurse Qualification (RGN) and valid NMC Pin.
  • Clearance: Clear DBS.
  • Experience: Proven track record of delivering person-centered care, including care planning.
