We are seeking a skilled Associate Practitioner to join our Crisis Resolution Home Treatment Team at Southern Health NHS Foundation Trust. As an Associate Practitioner, you will play a vital role in providing support to service users in crisis, working as an alternative to hospital admission.

Main Responsibilities

  • Contribute to the ongoing assessment, planning, delivery, and review of activities and interventions against identified health and wellbeing needs.
  • Organise and participate in defined assessment methods/activities.
  • Arrange care review meetings in line with Trust policy.
  • Develop and review appropriate care plans, identifying and organising other resources as appropriate.
  • Compute relevant paper and electronic documentation in line with Trust Record Keeping Policies.
  • Support service users to engage with their individual care plan, working in collaboration with both service user and their networks of support.
  • Provide support and therapeutic interventions to service users in order to promote their recovery in their community environment.

Person Specification

  • Foundation degree or NVQ in Health and Social Care level 3 or be working towards completion within next 3 months. Or high level of experience in a similar role.
  • Proven knowledge and demonstrable skills in caring for people with mental ill health.
  • An understanding of issues relating to the conditions of mental illness.
  • Experience of working with mental ill health.
