About the Role

We are seeking a dedicated Mental Health Social Worker to join our Caerphilly Mental Health Team. As a key member of our team, you will work collaboratively with individuals, families, and other healthcare professionals to provide high-quality assessments, interventions, and support.

Key Responsibilities
  • Conduct thorough assessments of individuals' mental health needs and risks.
  • Develop and implement person-centred care plans in collaboration with service users and their families.
  • Provide ongoing support and intervention to help individuals manage their mental health and achieve their goals.
  • Work closely with other health and social care professionals to deliver integrated care.
  • Engage in safeguarding processes to protect vulnerable individuals and ensure their safety.
  • Maintain accurate case records and documentation in line with relevant policies and procedures.
  • Advocate for service users, ensuring their rights and preferences are respected.
Requirements
  • Qualified Social Worker (Degree in Social Work or equivalent).
  • Registration with Social Work England (or equivalent).
  • Experience in mental health social work and knowledge of relevant legislation, including the Mental Health Act.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work effectively in a multi-disciplinary team environment.
  • A commitment to promoting diversity, equality, and inclusion in your practice.
  • Empathy and resilience, with a focus on recovery-oriented approaches.
