Job Summary

The Park Avenue Medical Centre is seeking an experienced Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. Key ResponsibilitiesClinical Practice
  • Conduct holistic clinical assessments and develop comprehensive management plans for patients with acute and chronic physical and mental health conditions.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions.
  • Prescribe and review medication, ensuring evidence-based practice and adherence to national and practice protocols.
  • Work with patients to promote compliance with and adherence to prescribed treatments, providing information and advice on medication regimens, side effects, and interactions.
  • Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
Communication
  • Utilize sensitive communication styles to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognizing the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
Delivering a Quality Service
  • Recognize and work within your own competence and professional code of conduct, delivering evidence-based care.
  • Comply with the NMC revalidation process and participate in the maintenance of quality governance systems and processes.
  • Produce accurate, contemporaneous, and complete records of patient consultations, consistent with legislation, policies, and procedures.
Leadership, Personal, and People Development
  • Act as a clinical leader, providing daily support to the duty team and Practice Management in the delivery of same-day services.
  • Problem-solve clinical matters arising and support the nursing team to deliver a quality service, communicating and providing feedback to Directors.
Requirements

To be successful in this role, you will need:

  • Registration with a professional body (NMC/HCPC).
  • Evidence of education at level 7 or higher.
  • Non-Medical Prescriber qualification.
  • Experience of working clinically as an Advanced Practitioner in Primary or Emergency care.
