NHS Birmingham and Solihull Integrated Care Board has an exciting opportunity for a motivated and enthusiastic pharmacist to join the Medicines Management and Optimisation Team as a Diabetes Pharmacist.
About the RoleThe post holder will work within allocated GP practices to support the implementation of the MMO strategy to promote high-quality, evidence-based, safe, and cost-effective prescribing. The successful candidate will have a strong clinical background, ideally with experience in diabetes, but training to develop into the role will be offered.
Main Duties and Responsibilities- Work as part of the multidisciplinary diabetes team in primary care to develop a tailored management plan for identified patients with diabetes.
- Use expertise to provide evidence-based prescribing advice to ensure effective, appropriate, cost-effective, and safe use of medicines and promote a patient-centred medicines optimisation agenda for patients with diabetes.
- Undertake clinical medication reviews for each allocated patient, utilising GP computer systems and discuss findings at the MDT to ensure required actions can be built into the care plan.
- Facilitate good prescribing practice in diabetes across the primary and secondary care interface.
- Deal directly with patients/carers as required, to improve concordance with diabetes treatment in order to achieve better health outcomes.
- Work with the ICS commissioning team to monitor the delivery of the diabetes aspects of the primary care contract by practices.
- Actively contribute towards management of the primary care prescribing budget through development of diabetes-related projects.

RequirementsThe post holder will need to have:
- Pharmacy degree
- Registered with General Pharmaceutical Council
- Relevant clinical diploma/postgraduate qualification
- Evidence of commitment to CPD
- Specialist diabetes expertise and significant post-registration experience
Desirable qualifications include independent prescribing qualification, knowledge of Primary Care, and involvement in education of others.
