About the Role
NHS Birmingham and Solihull Integrated Care Board has an exciting opportunity for a motivated and enthusiastic pharmacist to join the Medicines Management and Optimisation Team as a Diabetes Pharmacist.
The post holder will work within allocated GP practices to support the implementation of the MMO strategy to promote high-quality, evidence-based, safe, and cost-effective prescribing.
As a member of the MMO Team, patients will be at the heart of everything you do - you will be joining a team dedicated to delivering high-quality care and tackling health inequalities.
Main Responsibilities
- Work as part of the multidisciplinary diabetes team in primary care to develop a tailored management plan for identified patients with diabetes.
- Use expertise to provide evidence-based prescribing advice to ensure effective, appropriate, cost-effective, and safe use of medicines and promote a patient-centred medicines optimisation agenda for patients with diabetes.
- Undertake a clinical medication review for each allocated patient, utilising GP computer systems and discuss findings at the MDT so that the required actions can be built into the care plan.
- Facilitate good prescribing practice in diabetes across the primary and secondary care interface.
- Deal directly with patients/carers as required, to improve concordance with diabetes treatment in order to achieve better health outcomes.
- Work with the ICS commissioning team to monitor the delivery of the diabetes aspects of the primary care contract by practices.
- Actively contribute towards management of the primary care prescribing budget through development of diabetes-related projects.

The MMO team have core operational hours between 8am - 6pm, Monday to Friday.
