Clinical Pharmacist for GP Practice Specialist

We are seeking an experienced Clinical Pharmacist to join our team on a contractual basis, offering the opportunity to work remotely and manage your schedule. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.
Key Responsibilities
• Conduct comprehensive medication reviews to optimize patient treatment plans
• Review and manage acute medication requests with a focus on patient safety and efficacy
• Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions
• Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care
• Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes
• Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care
