Key Responsibilities

The successful candidate will maintain a prominent presence within the Trust and play a vital role in executing the Patient Safety strategy.

This role entails supporting the prompt and effective management of patient safety incidents by collaborating proactively with Investigation Teams and Divisional staff to ensure comprehensive responses that adhere to Trust, National, and Commissioner policies.

The individual will operate independently as a patient safety analyst while working alongside incident investigators and subject matter experts, promptly escalating any identified risks and immediate safety measures to the Head of Patient Safety Improvement.

Responsibilities include overseeing the daily management of patient safety incident responses, fostering a systems learning approach, and ensuring that patients, their families, Trust staff, and relevant third parties are actively engaged in the review process, all while exemplifying a commitment to continuous improvement.

Job Duties

The post holder will assist the patient safety team in upholding the patient safety incident response protocol within the Trust, ensuring compliance with established policies and best practices.

Responsibilities include:

  • Contributing to the enhancement of a systems learning approach and cultural shifts associated with the Patient Safety Incident Response Framework (PSIRF).
  • Reviewing patient safety incidents reported through the Trust's incident reporting system and other insights, including Freedom to Speak Up, complaints, and ward visits.
  • Conducting patient safety incident reviews using various methodologies, such as After Action Reviews, Clinical Reviews, and Structured Judgement Reviews.
  • Ensuring adherence to all internal and external reporting requirements and timelines for managing reported incidents.
  • Implementing the Duty of Candour in accordance with Trust and National Policy, while maintaining audit data to demonstrate compliance.
  • Promoting a just and learning culture across diverse clinical services and recognizing when specialized expert input is necessary.
  • Actively supporting Multidisciplinary Investigation Teams throughout the incident response process and identifying opportunities for learning and improvement.
  • Collaborating with Divisional colleagues to facilitate knowledge sharing and maximize opportunities for insight triangulation to enhance safety across the organization.
  • Working with patient safety specialists to maintain efficient systems and processes that ensure good governance and enable effective data collection, reporting, and statistical analysis.
  • Assisting in the development of the Trust's Patient Safety Incident Response Plan (PSIRP) and collaborating with various staff groups and community members to facilitate informed decision-making.

Required Skills

Essential

  • Proficient in writing professional reports that consolidate complex information from diverse sources.
  • Ability to engage with individuals from various backgrounds in emotionally charged situations while remaining composed.
  • Clinical knowledge and practical application of frameworks for incident review.
  • Strong negotiation and time management skills, with a proven ability to meet deadlines.
  • Capable of maintaining comprehensive records.

Knowledge and Experience

Essential

  • Experience in a healthcare environment.
  • Background in investigating patient safety incidents using systems-based investigation tools and human factors.
  • In-depth understanding of the Duty of Candour process.
  • Experience working within multidisciplinary teams.

Desirable

  • Proven track record of translating lessons learned from adverse incidents into actionable steps to minimize harm to patients and staff.

Education, Training, and Qualifications

Essential

  • Degree-level education or equivalent training and experience.
  • Recognized clinical registration (NMC, HCP).
