Job Title: Senior Practitioner - Learning Disability and Autism

We are seeking a highly skilled and experienced Senior Practitioner to join our innovative team at West London NHS Trust. As a key member of our Learning Disability and Autism (LDA) Community Forensic Team, you will play a vital role in supporting the discharge of service users from medium and low secure forensic mental health settings and their transition into community care.

Key Responsibilities:

  • Lead and monitor the performance and care delivery of nursing staff in the LDA Community Forensic Team
  • Participate in the review of clinical policies and guidelines relevant to the area and support and implement changes to practice
  • Undertake audit activity and supervise other staff in the service area/care group
  • Assess care needs and develop, implement, and evaluate programmes of care for a defined caseload, maintaining associated records
  • Support the team manager in providing effective leadership within a clinical team, ensuring a high-quality service within the resources available
