About the Role

We are seeking an experienced Harm Reduction Practitioner to join our team at Leeds and York Partnership NHS Foundation Trust. As a key member of our Complex Rehabilitation Service (CREST), you will play a vital role in supporting individuals with co-occurring mental health and addictions needs.

Key Responsibilities
  • Work closely with the multidisciplinary team to develop and implement individualized care plans that address substance misuse and harm reduction goals.
  • Provide direct and indirect support to service users, including assessment, engagement, and intervention planning.
  • Collaborate with service users, carers, and loved ones to ensure high-quality care is delivered.
  • Develop and maintain awareness and understanding of principles of harm reduction and substance misuse.
  • Contribute to the development of the service, including shaping the role of the Harm Reduction Practitioner.

    • Communication and Interpersonal Skills
    • Ability to work autonomously within scope of practice
    • Experience of working with clients with dual diagnosis
    • Experience of multi-agency working
    • Drug/Alcohol Qualification
    • Recognised professional qualification (RMN, RN (Mental Health))
