We are seeking an experienced General Practice Nurse to join our team at Morrab Surgery. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care to our patients, working closely with our GPs and other healthcare professionals to deliver evidence-based practice.
Key Responsibilities- Provide and maintain a high standard of nursing care to patients with long-term conditions and acute problems.
- Assess patient needs and provide appropriate healthcare, incorporating evidence-based practice.
- Develop and implement effective care management programs for frail and housebound patients.
- Support the Infection Prevention Control (IPC) lead and work to implement audits, training, and evidence of compliance.
- Develop and implement health promotion and wellbeing programs.
- Implement and evaluate individual treatment plans for chronic disease patients.
- Identify, manage, and support patients at risk of developing long-term conditions.
- Provide routine nursing care to patients, including diagnosing and managing acute and chronic conditions.
- Review patient medications and provide guidance on prescribed and over-the-counter medications.
- Coordinate the provision of travel medicine services and request pathology services as required.
- Maintain accurate clinical records and ensure compliance with practice policies.
- NMC registration.
- Primary care experience.
- Long-term condition management experience (Diabetes, Asthma, COPD).
