About the Role

We are seeking a highly motivated and compassionate Support Worker to join our Liaison and Diversion team in Preston. As a key member of our team, you will play a vital role in providing quality services for vulnerable individuals in contact with the Criminal Justice system.

Job Summary

This is a full-time, substantive post that requires successful candidates to undergo Level 2 (full) Vetting from Lancashire Constabulary. The successful candidate will be responsible for supporting access to health and social care services for potentially vulnerable adults and young people.

Key Responsibilities
  • Assess patient's individual care needs and refer to appropriate agencies.
  • Provide short-term intervention to encourage individuals to engage with services and attend initial appointments.
  • Complete and contribute to robust risk assessments and risk management plans.
  • Deliver and promote positive risk-taking to improve quality of life and maximize occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
Requirements
  • Education to Level 2 NVQ or equivalent experience.
  • Ability to navigate systems and electronic record-keeping systems.
  • Knowledge of the local community and the Criminal Justice system, including identified vulnerabilities.
  • Experience working with individuals with complex health and social care needs.
  • Experience of working in a community-based setting.
