Job Summary

This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our service users. As a Liaison and Diversion Support Worker, you will work closely with our Liaison and Diversion Practitioners to provide brief interventions and support individuals to engage with health and social care services.

Key Responsibilities
  • Assess patient's individual care needs and refer on to appropriate agencies.
  • Provide short-term interventions to encourage individuals to engage with services and attend initial appointments.
  • Complete and contribute to robust risk assessments and risk management plans.
  • Deliver and promote positive risk-taking to improve quality of life and maximize occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
Requirements

To be successful in this role, you will need:

  • Experience working with individuals with complex health and social care needs.
  • Knowledge of the local community and an understanding of the Criminal Justice system and identified vulnerabilities.
  • Awareness of a range of vulnerabilities and their impact on individuals.
  • Basic knowledge of signs and symptoms of mental disorders and their effects on individuals.
  • The ability to use electronic record-keeping systems.
  • The ability and skill to form positive therapeutic relationships, while maintaining professional boundaries.
