Job SummaryThis is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to make a real difference in the lives of our service users. As a Liaison and Diversion Support Worker, you will work closely with our Liaison and Diversion Practitioners to provide high-quality support and guidance to individuals in custody, court settings, and within the community.
Key Responsibilities- Assess patient's individual care needs and refer on to appropriate agencies.
- Provide short-term intervention to encourage individuals to engage with services and attend initial appointments.
- Complete and contribute to robust risk assessments and risk management plans.
- Deliver and promote positive risk-taking to improve quality of life and maximize occupational performance/independence.
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
- Education to Level 2 NVQ or equivalent experience.
- Ability to navigate systems and electronic record-keeping systems.
- Knowledge of the local community and an understanding of the Criminal Justice system and identified vulnerabilities.
- Experience working with individuals with complex health and social care needs.
- Experience and/or sound knowledge of the Criminal Justice System.
