About the Role
We are seeking a highly skilled and dedicated Physical Health Nurse to join our team at Elysium Healthcare. As a Physical Health Nurse, you will play a vital role in leading and coordinating the physical health needs of our service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team.
Key Responsibilities
- Monitor and assess the physical health needs of service users, identifying areas for improvement and implementing effective interventions.
- Develop and maintain strong relationships with primary health services, including GP practices and other physical health departments.
- Collaborate with the multidisciplinary team to ensure comprehensive care and support for service users.
- Contribute to the development and implementation of physical health interventions, using knowledge of current best evidence-based practice.
- Engage in practice development, evaluation, and audit activities to promote evidence-based practice and improve patient outcomes.
Requirements
- Relevant nursing qualification and registration.
- Phlebotomy skills.
- Strong team-working skills and a high level of self-motivation.
- A positive attitude and a commitment to high-quality care and values-based practice.
- Understanding of the physical health care needs of individuals with enduring mental health difficulties.

- Annual salary of £35,916 - £37,570.
