About the Role

We are seeking a highly skilled Physical Health Nurse to join our team at Elysium Healthcare. As a Physical Health Nurse Specialist, you will be responsible for leading and coordinating all aspects of the physical health needs of our service users.

Key Responsibilities

  • Monitor observations and assessments to identify any actions that need to be addressed and documented.
  • Develop and support physical health interventions used in the clinical area, adhering to the NMC Code of Conduct and local, Trust and national standards of practice.
  • Engage actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
  • Work collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
  • Monitor and maintain Physical Health Dashboards.

Requirements

  • A relevant nursing qualification and registration.
  • Phlebotomy skills.
  • Strong team-working skills.
  • A high level of self-motivation and a flexible approach.
  • A positive attitude and be naturally caring and compassionate.
  • A commitment to high-quality care and to values and evidence-based practice.
  • Understand the physical health care needs for individuals who are living with enduring mental health difficulties.
