Job Summary

The Central Surgery is seeking a highly skilled and motivated Advanced Clinical Practitioner Nurse/Paramedic to join our team. As an autonomous healthcare professional, you will work independently across our network, managing daily surgeries, online and telephone consultations, and providing holistic care to patients with a range of acute, non-acute, and chronic medical conditions.

Main Responsibilities
  • Work autonomously and be accountable for your professional actions.
  • Undertake clinical practice at an advanced level, using expert knowledge and clinical skills to deliver high-quality care to patients accessing primary care services.
  • Assess undifferentiated patients, take history, make physical examinations, formulate differential diagnoses, and carry out management treatment plans, ensuring relevant follow-up.
  • Discuss with registered GPs and make referrals to other healthcare professionals in primary and secondary care, as necessary.
  • Arrange hospital admissions for appropriate cases.
  • Undertake telephone triage work, telephone advice calls, and online consultations.
  • Manage and undertake home visits, including care home visits.
  • Manage palliative care patients.
  • Prescribe medication as necessary, as a non-medical prescriber.
  • Maintain accurate record-keeping in SystmOne/EMIS, using templates and READ codes.
  • Undertake regular medication reviews and medicines management.
  • Request and interpret tests appropriate to patients' needs.
  • Manage communications from hospitals and other providers, acting on requests for further action, medication, etc.
Person Specification
  • Desirable:
    • Experience working as an ACP.
    • Autonomous working.
  • Essential:
    • First-level nurse (RGN)/qualified paramedic.
    • Postgraduate diploma or degree in Advanced Clinical Practice.
    • Independent non-medical prescriber.
