Public Health Practitioner - 0-19 Healthy Child Programme

We are seeking a dedicated and compassionate Public Health Practitioner to join our dynamic team. In this role, you will play a vital part in promoting the health and well-being of children and young people aged 0-19 years.

Responsibilities:
  • Provide comprehensive health assessments and screenings to children and families.
  • Develop and implement individualized care plans based on identified needs.
  • Educate families on a range of health topics, including nutrition, immunizations, and developmental milestones.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Advocate for the health and well-being of children and families within the community.
  • Participate in public health initiatives and programs aimed at improving child health outcomes.
Qualifications and Experience:

The ideal candidate will possess:

  • Current NMC Registration
  • Experience working in a multidisciplinary setting
  • Knowledge of local and national policies informing the delivery of the Healthy Child Programme
  • Excellent communication, interpersonal, and problem-solving skills

Desirable Qualifications:

  • Mentor/998 or equivalent teaching qualification
  • Evidence of Degree level study in the last 3 years (or willingness to works towards)
  • Experience of Mentoring
  • Experience of working in the community
  • Experience working with children and their families
  • Experience with delivering healthcare interventions via digital platforms
  • Ability to use SystmOne
