Job Title: Residential Team Leader

Voyage Care is seeking a highly skilled and dedicated Residential Team Leader to join our team at our ABI Residential service in Bramley, Leeds.

About the Role:

As a Residential Team Leader, you will be responsible for supporting the Service and Deputy Manager in ensuring the delivery of high-quality care and support to individuals with complex needs. You will be the point of contact for the team when the Locality Manager is off-site and will be responsible for carrying out care needs assessments, creating and maintaining person-centered support plans, and ensuring that high standards of care and support are delivered.

Key Responsibilities:
  • Develop and implement individual support plans, including assessments and risk assessments.
  • Encourage individuals to take an active role in daily living and ensure the appropriate level of care and support is identified.
  • Manage the review and maintenance of support plans to ensure ongoing suitability.
  • Assist with personal care, including bathing, and maintain a high standard of hygiene.
  • Participate in planning and arranging daily outings, holidays, and recreational pursuits.
  • Build working relationships and engage in positive communication with individuals, families, advocates, and professional colleagues.
  • Undertake and record documentation to ensure accurate and informative records.
  • Ensure compliance with medication policies and staff medication competencies.
  • Coordinate and complete supervision, appraisal, and team meetings.
  • Attend and participate in multi-disciplinary meetings.
Requirements:
