Your Responsibilities:
- Provide professional leadership in complex case management, ensuring thorough assessment and support planning in high-risk situations.
- Prepare concise and informative written and verbal reports based on detailed analysis of complex evidence.
- Facilitate comprehensive support planning with service users to achieve identified outcomes through tailored support, resources, and enabling programs.
- Engage actively with caregivers, offering guidance and conducting assessments to empower them in their roles.
- Allocate and authorize appropriate support packages while managing limited budgets to commission necessary services.
