Job Title: Recovery Support Worker – Case Manager

Job Summary:

We are seeking a highly motivated and empathetic Recovery Support Worker – Case Manager to join our team at The Nelson Trust. As a key member of our residential addiction treatment team, you will play a vital role in supporting clients to achieve long-term change and develop their recovery capital.

Key Responsibilities:

  • Provide one-to-one support to clients in treatment, ensuring their individual needs are understood and met through the care planning process.
  • Facilitate a coordinated package of care for each client, working closely with other team members to ensure seamless support.
  • Work independently, with confidence and autonomy, to seek and provide support to colleagues as needed.
  • Contribute to the development and implementation of care plans, ensuring they are tailored to meet the unique needs of each client.
  • Participate in regular team meetings and contribute to the growth and development of the organization.

Requirements:

  • Experience of working within a residential addiction treatment setting.
  • Empathy and understanding of the challenges faced by clients in recovery.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including weekends and bank holidays.
