Job Summary:
We are seeking a highly skilled and dedicated Geriatric Medicine Specialist to join our team at NHS Professionals. As a Geriatric Medicine Specialist, you will be responsible for providing high-quality medical care to elderly patients, conducting comprehensive geriatric assessments, and developing individualized care plans to optimize function and quality of life.
Key Responsibilities:
- Conduct comprehensive geriatric assessments to evaluate the medical, psychological, and functional status of older adults.
- Diagnose and manage acute and chronic medical conditions commonly seen in elderly patients, such as dementia, frailty, falls, polypharmacy, and multiple comorbidities.
- Develop and implement individualized care plans that address the unique needs and preferences of each patient.
- Provide medical care for elderly patients admitted to hospital wards, including those in geriatric units, rehabilitation units, and general medical wards.
- Participate in daily ward rounds, updating care plans and ensuring appropriate follow-up for patients' conditions.
- Manage complex cases and coordinate with other medical specialties as needed.
Requirements:
- Medical degree and relevant postgraduate qualifications.
- Experience in geriatric medicine or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Knowledge of geriatric care principles and practices.
