We are seeking an experienced Care Coordinator to oversee our operations. This role is primarily office-based and involves various administrative responsibilities, including managing client files, preparing compliance documentation, and being available for on-call duties as needed. You will collaborate with healthcare professionals, care staff, clients, and their families to uphold high standards of care.
Reporting To: Care Manager
Role Summary:
The primary focus of this position is to ensure that all client visits are scheduled, recruit qualified Care Assistants, and maintain efficient office operations.
Main Responsibilities:
- Handle all incoming phone inquiries.
- Implement comprehensive recruitment and vetting processes for Care Assistants.
- Process new client referrals.
- Prepare necessary documentation for Field Care Supervisors.
- Ensure permanent assignment of Care Assistants to client visits, guaranteeing coverage at all times.
- Administer all Individual Care & Support Agreements.
- Maintain accurate and up-to-date client and Care Assistant files.
- Keep the computer system current at all times.
- Ensure all documentation and electronic records are consistently updated.
- Collaborate closely with the Field Care Supervisor.
- Ensure compliance with all office systems and procedures.
- Assist with payroll and invoice processing.
- Generate reports and management information as required.

- Excellent communication skills in English (both written and spoken)
