Job Summary

We are seeking a highly motivated and experienced Advanced Clinical Practitioner to join our Community Mental Health Services team in North Shropshire. As a key member of our team, you will be responsible for delivering high-quality, evidence-based care to patients in the community.

Key Responsibilities
  • Provide autonomous clinical assessment and treatment to patients with physical and mental health needs.
  • Contribute to the development and delivery of quality improvement programs.
  • Provide clinical leadership and management to ensure safe, quality, and effective care.
  • Develop and deliver education and training programs for healthcare professionals.
  • Lead on improvements in care quality and safety through service development, evaluation, and audit.
Requirements
  • Hold a Master's level award with appropriate modules in Advanced Health Assessment and Non-Medical Prescribing.
  • Take responsibility for own learning and development, recognizing and taking advantage of opportunities to learn.
  • Provide expert-level practice, leading to improved patient outcomes.
