Job Summary

An exciting opportunity exists for an experienced Registered Band 6 Mental Health Practitioner to work within a dynamic multi-disciplinary team, providing Community Rehabilitation Services to the Mental Health and Learning Disability Division for the areas of North Wales as part of Specialist Commissioning Care Services.

Main Responsibilities

The team is based in the East area of Wrexham. The appointed candidate will hold a caseload and act as Care Coordinator under the Mental Health (Wales) Measures Framework, delivering rehabilitation-focused services for service users within NHS rehabilitation inpatient units, Out of Area placements such as independent hospitals, low secure units, specialist placements, and also within the community.

Key Objectives
  • To provide a focus on the repatriation of service users back to their home area.
  • To provide intensive community rehabilitation to enable Service Users with Complex Emotional Needs and/or Complex Rehabilitation needs to remain within their local area.
Requirements

The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.
