About the Role

We are seeking a highly skilled and motivated Community Healthcare Specialist to join our Rapid Response and Intermediate Care (RRIC) team in Aylesbury. As a key member of our team, you will play a vital role in providing expert interventions and rehabilitation to patients in their own homes, empowering recovery and preventing unnecessary hospitalization.

Key Responsibilities
  • Provide comprehensive specialist assessment, review, case management, and evaluation of patient needs and carers.
  • Develop and implement personalized care plans to promote health gain and maximize independence.
  • Collaborate with a range of professionals to ensure timely and effective care delivery.
  • Lead and manage the RRIC team, allocating workloads and ensuring efficient case management.
  • Contribute to the development of clinical pathways and best practices within the team.

Requirements
  • Registered Allied Healthcare Professional with HCPC registration.
  • Valid and current driving license and car for travel throughout Buckinghamshire.
  • Experience of working in a community or in-patient setting, with a minimum of 2 years post-registration experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and colleagues.
