Community Healthcare Specialist

We are seeking a highly skilled and motivated Community Healthcare Specialist to join our Rapid Response and Intermediate Care (RRIC) team in Aylesbury. As a key member of our team, you will play a vital role in providing expert interventions and rehabilitation to patients in their own homes, empowering recovery and preventing unnecessary hospitalization.
Key Responsibilities- Provide comprehensive specialist assessment, review, case management, and evaluation of patient needs and carers.
- Develop and implement personalized care plans to promote health gain and maximize independence.
- Collaborate with a range of professionals to ensure timely and effective care delivery.
- Lead and manage the RRIC team, allocating workloads and ensuring efficient case management.
- Support the development of clinical pathways and contribute to the improvement of patient outcomes.
