About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our Rapid Response & Intermediate Care Team in the community. As a key member of our team, you will provide high-quality care and support to patients in their own homes, promoting independence and well-being.

Key Responsibilities
  • Deliver person-centered care to patients, including nursing, social, and rehabilitative interventions.
  • Work collaboratively with the Rapid Response & Intermediate Care Team to achieve timely discharges and avoid hospital admissions.
  • Conduct assessments and develop care plans to meet individual patient needs.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Participate in multidisciplinary team meetings to discuss patient progress and develop care plans.
Requirements
  • Previous experience in a healthcare setting, preferably in a community or nursing home environment.
  • Good communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Valid driving license and access to a car for work purposes.
