We are seeking a highly skilled and experienced Clinical Pharmacist to join our dynamic team at Horizon Healthcare. Key Responsibilities- Develop and maintain good working relationships with all relevant staff involved in providing healthcare across Southern Derbyshire.
- Develop and maintain good relationships with all patients encountered through the role.
- Lead delegated areas within the delivery of GP clinical pharmacy by managing and developing and overseeing systems and processes to deliver the service.
- Maintain registration as a pharmacist and comply with professional codes using own caseload to maintain pharmacy competence.
- Maintain personal indemnity insurance including vicarious liability cover at the practice.
- Plan and organise agreed practice workload with the practice.
- Comply with all organisational and statutory requirements (health and safety, equal treatment and diversity, confidentiality and clinical governance).
- Undertake any other duties commensurate with the post holder's grade and experience as agreed with the GP practice.
Horizon Healthcare has a list size of 24,000 plus patients, who are served by multi-disciplinary teams of both administrative and healthcare professional colleagues, of which the Pharmacy team plays an integral part. We offer competitive rates of pay based on experience and skill levels, as well as flexibility around start/finish times.
Requirements- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (patients).
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Good IT skills.
- Able to obtain and analyse complex technical information.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Able to work under pressure and to meet deadlines.
- Produce timely and informative reports.
- Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
- Minimum of 2 years post qualification experience.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- An appreciation of the nature of GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Masters degree in pharmacy (MPharm) (or equivalent).
- Independent prescriber.
- Mandatory registration with General Pharmaceutical Council.
