About the Role
We are seeking a highly skilled Clinical Pharmacist to join our dynamic team at Horizon Healthcare. As a Clinical Pharmacist, you will play a vital role in providing expert clinical pharmacy support to our practice, working closely with our multidisciplinary team to deliver high-quality patient care.
Key Responsibilities
- Develop and maintain good working relationships with all relevant staff involved in providing healthcare across Southern Derbyshire.
- Develop and maintain good relationships with all patients encountered through the role.
- Lead delegated areas within the delivery of GP clinical pharmacy by managing and developing and overseeing systems and processes to deliver the service.
- Maintain registration as a pharmacist and comply with professional codes using own caseload to maintain pharmacy competence.
- Maintain personal indemnity insurance including vicarious liability cover at the practice.
- Plan and organise agreed practice workload with the practice.
- Comply with all organisational and statutory requirements (health and safety, equal treatment and diversity, confidentiality and clinical governance).
- Undertake any other duties commensurate with the post holder's grade and experience as agreed with the GP practice.

Horizon Healthcare has a list size of 24,000 plus patients, who are served by multi-disciplinary teams of both administrative and healthcare professional colleagues, of which the Pharmacy team plays an integral part. Person Specification
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (patients).
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Good IT skills.
- Able to obtain and analyse complex technical information.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Able to work under pressure and to meet deadlines.
- Produce timely and informative reports.
- Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
Desirable Qualifications and Experience
- Minimum of 2 years post qualification experience.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- An appreciation of the nature of GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Essential Qualifications
- Masters degree in pharmacy (MPharm) (or equivalent).
- Independent prescriber.
- Mandatory registration with General Pharmaceutical Council.
