About the Role

We are seeking a highly skilled Clinical Pharmacist to join our dynamic team at Horizon Healthcare. Key Responsibilities

  • Develop and maintain good working relationships with all relevant staff involved in providing healthcare across Southern Derbyshire.
  • Develop and maintain good relationships with all patients encountered through the role.
  • Lead delegated areas within the delivery of GP clinical pharmacy by managing and developing and overseeing systems and processes to deliver the service.
  • Maintain registration as a pharmacist and comply with professional codes using own caseload to maintain pharmacy competence.
  • Maintain personal indemnity insurance including vicarious liability cover at the practice.
  • Plan and organise agreed practice workload with the practice.
  • Comply with all organisational and statutory requirements (health and safety, equal treatment and diversity, confidentiality and clinical governance).
  • Undertake any other duties commensurate with the post holder's grade and experience as agreed with the GP practice.

Horizon Healthcare has a list size of 24,000 plus patients, who are served by multi-disciplinary teams of both administrative and healthcare professional colleagues, of which the Pharmacy team plays an integral part. Person Specification

  • Excellent interpersonal, influencing and negotiating skills.
  • Excellent written and verbal communication skills.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (patients).
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Good IT skills.
  • Able to obtain and analyse complex technical information.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
  • Able to work under pressure and to meet deadlines.
  • Produce timely and informative reports.
  • Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
  • Work effectively independently and as a team member.
  • Demonstrates accountability for delivering professional expertise and direct service provision.

Desirable Qualifications and Experience

  • Minimum of 2 years post qualification experience.
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.

Essential Qualifications

  • Masters degree in pharmacy (MPharm) (or equivalent).
  • Independent prescriber.
  • Mandatory registration with General Pharmaceutical Council.
