Job Title: Team Leader

We are seeking an experienced Team Leader to join our team and contribute to the delivery of high-quality supported housing schemes and domiciliary services in Blackpool.

Key Responsibilities:
  • Manage a team of staff to ensure the provision of person-centred support, enabling customers to maintain their independence and experience improved wellbeing.
  • Supervise, coach, and support staff, volunteers, and students on placement to ensure they meet individual needs and preferences.
  • Identify training needs and ensure accurate records are kept.
  • Respond to referrals and undertake initial assessments of need.
  • Ensure all service users have a co-produced plan of support and a designated Key Worker.
  • Develop clear goals and expected outcomes for service users, with a defined pathway towards greater independence.
  • Coordinate regular person-centred reviews, prioritizing each person's views and unique journey.
  • Collaborate with local multi-disciplinary teams and community-based agencies to provide holistic, wrap-around support.
  • Maintain high standards of environmental safety and quality, ensuring a welcoming and inclusive atmosphere.
  • Foster service user engagement, involvement, and peer support.
  • Implement robust quality monitoring methods.
  • Provide key performance indicators and plan actions required.
Requirements:
  • Strong leadership and management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with stakeholders.
  • Knowledge of relevant legislation and policies.
  • Experience in supported housing or domiciliary care.
