Job Overview

We are seeking a highly skilled and compassionate Senior Mental Health Practitioner to join our CAMHS Rapid Response team in Reading. As a key member of our team, you will play a vital role in providing high-quality mental health assessments and short-term interventions to young people experiencing mental health crises.

Main Responsibilities
  • Conduct comprehensive psychosocial crisis assessments for young people who have presented in crisis.
  • Offer short-term intervention to stabilise the crisis, including follow-up appointments and multi-agency consultation-liaison.
  • Identify ongoing care plan needs for young people and their families, ensuring they are referred to appropriate services.
  • Maintain and develop key relationships with professionals external to the area of service, as well as patients and their families.
  • Accurately maintain clinical record keeping and ensure all systems, records, and data are stored and transferred correctly.

Requirements
  • A degree in a relevant field, as well as a core profession (e.g., Social worker, Nurse, OT).
  • Experience in working with mental health and complex needs.
  • Demonstrable skills in mental health assessments and formulation.
  • A full UK driving license with access to a vehicle.
