About the Role

Nurslink is seeking a skilled and dedicated Registered Community Nurse to join our team. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Collaborate with Patients and Families: Work closely with patients, families, and multidisciplinary teams to develop and implement personalized care plans.
  • Provide Skilled Nursing Care: Administer medication, manage symptoms, and provide wound care to patients in their own homes.
  • Coordinate Care and Liaise with Healthcare Professionals: Communicate effectively with other healthcare professionals and community services to ensure seamless care delivery.
  • Maintain Accurate Documentation: Record and update patient assessments, care plans, and interventions in a timely and accurate manner.
  • Develop Strong Clinical Assessment and Communication Skills: Conduct thorough assessments and communicate effectively with patients, families, and healthcare professionals.
Requirements
  • Registered Nurse with Current NMC Pin: Possess a valid nursing qualification and current registration with the NMC.
  • Valid Driving Licence and Access to a Vehicle: Hold a valid driving licence and have access to a vehicle for work purposes.
  • Relevant Nursing Qualifications: Possess relevant nursing qualifications and experience.
  • 6 Months Recent Experience as a Community Nurse: Have at least 6 months of recent experience working as a Community Nurse.
  • 2 Clinical References: Provide 2 clinical references that cover 3 years of work experience.
  • Valid DBS Certificate: Hold a valid DBS certificate.
