About the Role

We are seeking experienced Community Nurses to join our team at Nurslink. As a Community Nurse with us, you will have the opportunity to work with a dedicated team of professionals who are passionate about delivering high-quality care to patients in the Leicester area.

Key Responsibilities
  • Collaborative Care Planning: Work with patients, families, and multidisciplinary teams to develop and implement personalized care plans.
  • Skilled Nursing Care: Provide wound care, medication administration, and symptom management to patients in their homes.
  • Coordination and Liaison: Coordinate care with other healthcare professionals and community services to ensure seamless delivery of care.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date documentation of assessments, care plans, and interventions.
  • Excellent Communication Skills: Develop and maintain strong relationships with patients, families, and healthcare professionals through effective communication and interpersonal skills.
Requirements
  • Qualifications: Registered Nurse with a current NMC pin and relevant nursing qualifications.
  • Experience: At least 6 months of recent experience working as a Community Nurse.
  • References: Two clinical references that cover 3 years of work experience.
  • DBS Certificate: Valid DBS certificate.
  • Driving License: Must have a valid driving license and access to a vehicle.
