Position Overview

Role: Associate Practitioner - Band 4

The successful candidate will join the Rapid Response/Virtual Ward team, playing a crucial role in supporting patients to remain in their homes while utilizing advanced technology to enhance their well-being.

This role operates within established parameters of Associate Practitioner competencies, executing specific clinical tasks and responsibilities as delegated by registered nursing or therapy professionals.

The Associate Practitioner will demonstrate proficiency that exceeds the expectations of a Band 3 Health Care Support Worker, collaborating effectively with team members, service users, general practitioners, multidisciplinary teams, and private or voluntary organizations.

The individual will provide essential support to Occupational Therapists, Physiotherapists, and Rapid Response Matrons, undertaking assigned duties from qualified nursing or therapy staff.

Autonomy is a key aspect of this role, as the post holder will work in diverse environments, including patients' homes and various community settings, such as intermediate care facilities and care homes.

Main Responsibilities

The Associate Practitioner will be an integral part of the team, facilitating the service's mission to enable unwell patients to stay at home and reach their maximum potential. Responsibilities include:

  • Working within defined Associate Practitioner competencies.
  • Carrying out delegated clinical tasks as instructed by registered professionals.
  • Collaborating with multidisciplinary teams and external agencies.

Qualifications and Experience

Essential:

  • Foundation degree in Healthcare practice.
  • A minimum of two years' experience in a community setting as a Band 3 Healthcare or therapy assistant.

Knowledge and Skills

Essential:

  • Understanding of multidisciplinary team roles.
  • Awareness of confidentiality and its significance.
  • Knowledge of equality and diversity principles.
  • Familiarity with child and adult protection protocols.

Desirable:

  • Experience with Telehealth and remote patient monitoring.

Skills and Abilities

Essential:

  • Ability to work independently and recognize personal limitations.
  • Basic nursing and therapy skills.
  • Strong communication and listening abilities.
  • Effective organizational and observational skills.
  • Capacity to work collaboratively within a multidisciplinary team.
  • Ability to manage stress and maintain composure in emergencies.
  • Proficiency in maintaining accurate patient documentation.

  • Valid driver's license and access to a vehicle for work-related travel.
  • Basic IT proficiency.
