An exciting opportunity has risen to recruit a full time Band 4 as a MDT Coordinator and Patient Pathway Coordinator at the Children’s Hospital Unit - Royal National Orthopaedic Hospital Trust.

The MDT coordinator will work as an integral member of the multi-disciplinary teams, providing a pro-active and facilitative clinical pathway navigation service by supporting clinical teams in ensuring that patient care is delivered in an effective and timely manner in accordance to national guidelines.

The post holder will also provide administrative support to the children’s hospital unit and provide a professional, efficient administrative service to patients.

While the post holder will be formally identified as an MDT Coordinator and patient pathway coordinator, in recognition of their specific function within each Clinical Support Unit, this job description has been designed to function as a generic description for both roles. This is in recognition of the combined nature of the role in certain departments, and of the expectation of full cross-cover within unit teams. Local policies and processes will more closely define the day to day requirements of each role.

To be the main point of contact for all patients issues relating to their pathway of care.

To be responsible for the management of the patient pathway, from referral to first outpatient appointment, through diagnostics, treatment planning, treatment, and follow-up, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

To have a sound understanding of 18 week waiting time targets, rules and procedures, and to work proactively to ensure that all targets are maintained.

To ensure high levels of patient and clinician satisfaction by being an accessible, customer focussed and knowledgeable point of contact.

The Royal National Orthopaedic Hospital is a specialist tertiary referral hospital. A fifth of our patients are younger than 18 years of age. Patients are referred to the hospital nationally and internationally, often with complex medical needs.

**Children are admitted under different teams including**:
Paediatric Orthopaedics

Spinal Injuries

Spinal Deformity

Spinal Malformation

Bone and Soft Tissue tumours

Peripheral Nerve Injury

Paediatric Medicine (Metabolic Bone &Rheumatology)

The team consists of 5 secretaries in Paediatric Surgery and 2 in Paediatric Medicine. It is a supportive team that provides cover for each other.

To provide a consistent approach across the Trust, Standard Operating Procedures (SOP) will describe how the functions of this role will be carried out along with the timescales to which are to be adhered.

Provide comprehensive secretarial/PA support to consultants and clinical teams within the Children’s Hospital Unit.

To routinely attend performance review meetings and ensure that agreed actions and outcomes are requested, documented and monitored.

To be responsible for booking and sending out-patient, admission appointment letters and telephoning patients as required.

To schedule outpatients appointments and ensure clinics are managed effectively.

To attend weekly planning meetings and maintain strong communication links.

Deal with non-clinical queries from patients, their carers, general practitioners and members of the admin and clerical team.

To take overall responsibility for the waiting list and monitor the quality of patient pathways using Trust IT system.
