**Our Network teams include**:

- Enhanced Care Visiting team for frail housebound patients and those in care homes. The team includes visiting GPs, ANPs, Paramedic, Frailty Nurses, Care Coordinators, Pharmacist.
- BEC Urgent Care Treatment Centre working out of the Private Suite at Shelley Manor Medical Centre.
- First Contact Physiotherapy Practitioner service.
- Digital Care Coordinator, Digital Champions.
- Mental Health practitioners.
- Health & Wellbeing Coaches within Help & Care team.

**The role**

We are recruiting for **a DIGITAL CARE CO-ORDINATOR **to assist in the smooth running of digital transformations for the PCN, on a part time, permanent contract working 24 hours per week.

Digital Care Coordinators play a vital part in the digital transformations happening across primary care. In support of collaborative working, the Digital Care Coordinator will form productive and supportive relationships with our practice and PCN staff as well as with neighbouring Digital Care Coordinators which will prove helpful when rolling out new services across the network.

**Main duties and responsibilities (this list is not exhaustive)**:

- To develop a continuous improvement to digital accessibility and digital maturity.
- Assisting the PCN Digital Lead with engaging our patient population and healthcare professionals to utilise the potential of digital interventions and technology in healthcare.
- The DCCO will assist in leading on CVD management across the network.

**Digital**
- BEC hypertension pathway - actively monitor patients referred to this service, follow the PCN hypertension guide to ensure this service is delivered promptly. Assist the Digital Lead in the roll out and maintenance of this service to all practices of the PCN.
- Support the PCN Digital Lead in rolling out/maintenance of our PCN’s blood pressure at home pathway and the setup/maintenance of future remote digital healthcare services and provide any training if required.
- Assist PCN Digital Lead to enhance online access for patients. Work with digital volunteers, provide training as necessary for digital drop-in sessions, digital clinics and any further digital interventions.
- Involve yourself into the existing community of Digital Care Coordinators, networking with neighbouring PCN’s.
- Support Digital Lead with any digital campaigns.
- Support the PCN Digital Lead with new and ongoing projects, e.g., SystmOne in care homes. Run various system reports to identify areas of focus and support Digital Lead with any required audits.

Job responsibilities include, but are not limited to:
**Information Technology**
- Troubleshoot ad hoc staff and patient queries relating to technology in healthcare at home, e.g., using SystmOnline questionnaires.
- Produce guidance and train staff to develop skills on S1 and other platforms/software used by the PCN as directed by the PCN management team.

Training will be provided in these areas, depending on experience.

**PERSON SPECIFICATION (this list is not exhaustive)**

**Essential criteria**:

- Grade 5 and above GCSE’s to include Maths and English.
- Strong communication in English, both written and spoken.
- Experience using and strong knowledge of Microsoft Office (Outlook, Word, PowerPoint and Excel).
- Analytical and project planning skills.
- Good coordination, organisational and time management skills.
- Ability to multitask, be a self-starter and take busy days in your stride.
- Demonstrate empathy and compassion where situations require.
- Excellent communication and telephone skills.
- Ability to work as part of a team and on own initiative.

**Desirable**
- IT / Digital / Administrative experience either in Primary Care or NHS setting.
- Have good in-depth knowledge of SystmOne - Dorset’s Primary Care clinical and administrative system.
