Would you like to be part of an exciting development that is taking place within our Dunstable community, within a new modern integrated health and social care hub?You will be part of The Luton North Community Mental Health Service (previously known as Phoenix Sunriser). This brand new development will be a 'one stop' hub where people can access high-quality care and services close to where they live making it accessible, convenient and easier for local residents to use. It will bring a mix of health and care professionals in a single location to promote more joined-up working between the NHS, the council, voluntary and community sectors.

The post holder will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the operational policy. The post holder will work as an autonomous practitioner within an integrated team. This will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the client. The post holder will be required to administer prescribed medication which includes intramuscular injections.

**The post holder will be responsible for**:

- Ensuring that the values outlined in the NHS Constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust. Responsible for delivering a compassionate, dignified and respectful service to patients at all times.
- Continually delivering and maintaining high standards for a defined caseload as agreed with Line Manager.
- Leadership, supervision and support to junior staff.
- Ensuring national targets and standards are achieved.
- Building trust and developing relationships to ensure service user involvement.
- Demonstrating a professional manner in all work carried out.
- Maintaining accurate and timely records.
- Ensuring equality and diversity values underpin the day to day working of the practice area.
- Continuous professional development.

This team is part of the Mental Health Transformation of services and our new blended team where our voluntary service colleagues are fully embedded within the team. This is definitely an exciting time to join this expanding, developing and modern service.

The team is also made up of a rich resource of multidisciplinary professionals who support psychological ways of working. The team includes Registered Nurses, Social Workers. Doctors, Psychologists, Assistant Psychologist, Occupational Therapists, Support Recovery Workers, Peer Support Worker, Psychotherapists, Administrators and an Employment specialist as well as working closely within a designated locality with Primary care mental health link workers, the Local Authorities and voluntary services.

You will work closely with various other services such as GP Practices, the local authority adult social care, substance misuse and children’s services to name but a few.
