Job Overview

We are seeking a highly skilled and experienced Band 6 Sister/Charge Nurse to join our team at Oxleas NHS Foundation Trust. As a key member of our community care team, you will be responsible for delivering high-quality patient-focused care to adults in their own homes.

Key Responsibilities
  • Assess patient care needs and develop individualized management plans using a problem-solving approach and evidence-based care.
  • Work collaboratively with community and in-house services to deliver an integrated care model.
  • Provide intensive medical treatment for acutely unwell patients in their own homes.
  • Facilitate patient discharge from hospital by providing additional support and care in the community.
Requirements
  • Qualified Nurse with a strong background in acute care.
  • Experience working as a Band 6 Sister/Charge Nurse or Band 5 with extensive nursing experience in an acute setting.
  • Ability to supervise junior staff and provide clinical supervision.
  • Strong communication and interpersonal skills.
  • Ability to work flexibly across a 24-hour day, including evenings and weekends.
