Job Overview

We are seeking a highly skilled and experienced Band 6 Sister/Charge Nurse to join our team in the Hospital at Home Service. As a key member of our nurse-led team, you will play a vital role in delivering high-quality, patient-focused care to adults in their own homes.

Key Responsibilities

As a Band 6 Sister/Charge Nurse, you will be responsible for assessing care needs, developing and implementing management plans, and providing exceptional care and treatment to patients in their own homes. You will work closely with our multidisciplinary team to ensure seamless care coordination and delivery.

Requirements
  • Qualified Nurse with a strong background in acute care
  • Experience working in a community or hospital setting
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to write reports and use IT systems
