Senior Clinical Practitioner

Job description Holistically assess and clinically examine patients who fall within your scope of competence, through history taking, physical examination and appropriate use of tools to confirm diagnosis. Plan, implement and evaluate an effective management plan for the patient with support from senior colleagues. If you are a non medical prescriber (NMP) you will provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. You will work towards your NMP qualification if you are not currently a NMP.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Maintain effective communication with senior clinical colleagues, the wider team, the organisation and with external stakeholders. Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
Non-Medical Prescribing You must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes. If you are not a NMP, you will work towards this qualification. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.
Delivering a Quality Service Recognise and work within own competence and professional code of conduct as regulated by the professional regulatory body. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
Ensure professional revalidation requirements are met. Deliver care according to NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
Contribute to the development of local guidelines, protocols and standards. Maintain effective communication with those responsible for the overall commissioning and procurement process. To attend and be proactively involved in professional meetings. Take the lead/ develop a special interest within certain areas to allow the sharing of clinical knowledge and expertise.
