Advanced Practitioner

A new role for an Advanced Practitioner has been made available at an exciting time for Oldham CMHT. We are working towards our 5 year Transformation plan and this role is to assist with our current stabilisation and bolster phase. The Transformation aims to bring about an integrated model of Primary, Secondary and Specialist mental health services to ensure our clients are supported through diagnosis, symptom management, risk management and recovery to enable people with mental health problems live a good life and recognise the role mental health services have in assisting clients with this. The Trust has made significant funding available to the CMHT to ensure the new model has a solid foundation to support this change.

The Post-holder will
- Be an experienced Nurse/Allied Health professional qualified to master’s level
- Undertake a significant clinical caseload to an advanced professional standard using specialist knowledge from theoretical and practical experience.
- Develop and extended scope of practice beyond their primary profession.
- Assess referrals from internal and external sources and make onward referrals.
- Work within the 4 pillars of Advance Practice and maintain a portfolio of evidence to enhance personal and professional development.
- Actively participate in planned developments working on projects, audits, elf-development and enhancing own clinical skills and knowledge
- Work collaboratively with the Consultant Psychiatrist to meet the needs of patients and play a key role in service delivery and adherence to policy and procedures.
- Play an active role in training junior staff and supporting professional colleagues
Pennine Care Foundation Trust covers Oldham, Tameside, Heywood, Rochdale, Stockport and Bury. We are developing a new community based offer that will include access to psychological therapists, improved physical health care, employment support, personalised and trauma informed care, medicines management and support for self-harm and coexisting substance use - and proactive work to address racial disparities. Our local areas will be supported to redesign and reorganise core community mental health teams to move towards a new place-based, multi-disciplinary service across health and social care aligned with primary care networks.
- To develop and demonstrate a high degree of competency and expertise in the provision of Community Mental Health.
- To be expert in the provision of evidence-based care.
- To provide clinical leadership for the CMHT.
- Demonstrate applied knowledge of mental health legislation, related codes of practice and national and local policy and guidance
- Ability to identify the presence of mental disorder, the severity of the disorder; and determine whether the disorder is of nature or degree warranting compulsory detention
- To advise and lead on clinical risk assessment and management and to ensure that risk assessments and management plans are updated regularly.
- To lead on cases that are complex in nature and support practitioners in their role as key workers for individuals referred into the service.
- Provide expert advice to clinical staff within the team and within other services on the assessment, planning, implementation and evaluation of care and treatment of service users.
- Provide expert advice to clinical staff within the team on the assessment and management of the physical health needs of service users
- To develop and exercise high levels of judgement, discretion and decision making in client care and management.
- To interface between nursing, allied health professionals and medical colleagues to ensure that service user needs are being met.
- Advise on the appropriateness and suitability of referrals into the service providing feedback to the referrer where appropriate.
- To ensure all areas of work of the post holder promotes service user and carer involvement and promotes high quality care and social inclusion.
- To develop extended practice/new ways of working that fits and supports the clinical core competencies and role of Advanced Practitioner. These Clinical Core Competencies will include:
