Pathway 3 Navigator

We are a new, expanding team looking for 2 enthusiastic team players to join us as P3 Navigators.
The post holders will provide coordinated care for patients with complex health needs, who are being transferred from an acute bed onto a discharge to assess (D2A) pathway 3. The post holders will work on behalf of the integrated multiagency hub team, to source and coordinate safe and timely access to pathway 3 nursing home beds.
This post is a critical part of the discharge to assess pathway.
To act as the main focal point for prioritising the communications within the acutes wards, multiagency discharge hubs, continuing healthcare and private nursing homes to ensure provision of seamless care to patients with complex care needs being discharged onto a discharge to assess pathway 3 provision.
To identify and provide the relevant trusted assessor information to pathway 3 providers to ensure they are fully aware of the patients needs so they can determine if they are able to care for them.
To offer comprehensive administrative support to the transfer of care hub team for any further discharge to assess pathways as required.
Build and maintain a wide network of relationships with pathway 3 providers and with support of the integrated hub managers, coordinate any learning from the pathway 3 discharges back to the acute wards and multiagency hub teams.
To respect confidentiality for work undertaken and can use their initiative, work with minimum supervision to deadlines and maintain a flexible approach.
